What is Medication Therapy Management (MTM)?

MTM is a meeting with the pharmacist to review ALL of your medications, allergies, and conditions. The pharmacist does follow-up with your physician to address all of your medication-related needs and concerns.

Eligibility

MTM is covered by your Medicare Part D prescription plan if you meet their eligibility criteria:

1. Taking multiple chronic medications AND

2. Have multiple chronic conditions AND

3. Spend more than $3,000 per year on covered Part D medications.

However, some private insurance companies may also cover MTM. Please check with the pharmacist to see if you are eligible.
